As You Begin Cancer Treatment
When you hear the words “You have cancer”, it is often difficult to hear or think about anything else. Likely what follows your diagnosis is a treatment plan that has been decided by you and your cancer doctor. These plans are typically specific to treating your cancer and either preventing or managing side effects. You may also have other health conditions that need to be monitored by your primary care physician. We will coordinate with them throughout your journey.
YOUR “MEDICAL HOME” AWAY FROM HOME
Our team will focus on your cancer care and work with your other health care providers to make sure all your needs are met. This is why your cancer care team will be your “medical home”. Your oncologist will lead this team by communicating and coordinating with all others involved in your care to help insure the best possible outcome.
An Oncology Medical Home is a nationally recognized team-based care model that puts YOU at the center. The purposes of a medical home are to: a) allow better access to health care, b) increase your satisfaction and medical outcomes and, c) deliver the care as economically as possible. This program begins with the first visit to your oncologist and will continue throughout your treatment and follow-up.
These standards for cancer care include:
